INTRODUCTION Postpneumonectomy syndrome (PPS) is a rare complication of a right pneumonectomy, occurring occasionally in a left pneumonectomy with a right aortic arch. It develops due to the displacement of mediastinal structures to the empty pleural space, rotating itself as it moves. The distal third of the trachea and the left main left bronchus are compressed by the left pulmonary artery anteriorly and by the thoracic vertebrae posteriorly, generating an anteroposterior occlusion.
