The discharge planning provides important information and individualized inpatient and / or their primary caregiver; care also includes: medicines, non-drug treatments, diet among others; reinforces and comprehensive care should be the person at home, provided by itself or by their primary caregiver, favoring a speedy recovery. Objective: Analyze the importance of the discharge planning from the perception of inpatient and / or their primary caregiver. Methodology: qualitative, phenomenological; IPS conducted in Cartagena de Indias; in hospitalized patients and / or their primary caregiver. The information was collected through a semi-structured interview. Results: El predominant sex caring at home is female, between the first and second degree of consanguinity, the perception of the hospitalized patient and primary caregiver threw four categories; the voices expressed that the lack of clarity in the discharge plan has consequences for the patient, psychological and physiological problems placing their health at risk with possible complications, generating waste of time and economic resources for the entities that provide health services; state that the discharge plan facilitates recovery and helps improve quality of life. Conclusion: inpatient and their primary caregiver is vital to the implementation of the discharge planning by the professional nurse at the time of hospital discharge, as they describe the recommendations gives them security in care who have to implement at home.
