Pneumocystis jirovecii pneumonia is a rare infection, but is a severe and life-threating complication in kidney transplantation patients. This infection occurring during the first six months after transplantation which is associated with high dose immunosuppressive therapy; however, it could present after six month and it is associated with stopping trimethoprim sulfamethoxazole prophylaxis. We report two cases of severe Pneumocystis jirovecii pneumonia ) , presented six months after kidney transplantation; they received treatment with trimethoprim sulfamethoxazole and steroids, one patient succumbed to the infection and the second patient presented a complete recovery with preserved kidney graft function).
