We report an unusual case of mediastinal abcessed mass, with involvement of the lung, thymus and probably bone and hematopoietic system, in a newborn that was diagnosed with Langerhans cell histiocytosis. Its interest lies in the presentation as a rare cause of mediastinal mass with involvement of other organs. Histologic examination of the surgical specimen demonstrated Langerhans cell histiocytosis. It is known that vascular and lymphatic malformations, with teratomas, neurogenic tumors, lymphadenopathy of infectious origin and mediastinitis, are a major cause of mediastinal masses; in this case, it must also take into account the Langerhans Cell Histiocytosis as a cause. It had already reported cases of congenital solid masses neck and thigh as a unique presentation of this disease.
