Introduction: Spontaneous perforation of biliary tract is an uncommon condition presented in children and adults. Pediatric patient usually has not a defined etiology. Clinical presentation is generally sub acute, with unspecific symptoms like abdominal pain, mild jaundice, abdominal distention, ascites or signs of peritonitis. Objectives: To describe a clinical case which details the typical presentation of this entity and to review the concerning literature. Results: An 8 year old male presented unspecific digestive symptoms and signs of acute abdomen. He did not have ultrasonogram or abdominal scan for preoperative diagnosis. At surgery, he showed a perforation at the junction of the cystic and common bile duct. Conclusion: Despite the low incidence of spontaneous perforation of the common bile duct, it is fundamental to know the pathogenesis and optimal management especially in cases of incidental intraoperative diagnosis.
