The ITIO (stands for Integration of Information Technologies in the Organizations, in Spanish) research group has developed, as part of a project by Politecnica de Valencia University, an application that allows the generation and completion of surveys via Internet, with the support of a database. In this article, the automatization of the survey generation and completion process is described, collecting the results in the database for their statistical analysis. The data can be easily accessed through the phpMyAdmin program or through other applications designed for the same purpose, allowing their modification and/or check. This survey system is currently being applied with the purpose of gathering information and allowing the selfevaluation of small and medium-sized industries, in sectors such as textile, furniture and farm loan banks, among others, to analyze the statistical data deriving from the application of EFQM (European Foundation for Quality Management) models and ICP (Integral Control Panel).
