We present a case of a 43 year old woman who presented to the emergency department with neurological symptoms given by severe headache and vomiting. At the begining the case was interpretated as a migraine but due to persistente headache and clinical deterioration neuroimages were performed showing cerebellar lesions sugestive of ischemic infarction with sings of rupture of the hematoencephalic barrier. In spite of medical treatment the clinical state deteriorated showing up systemic inflamatory reponse and refractory hyperglycemia. After somedays the patient presented cardiorespiratory failure and death. In the necropsy a pancreatic adenocarcinoma metastasic to brain was diagnosed.
