Introduction. Sun exposition is the main cause of skin aging.(1)There are many different classifications of sun face damage but it does not exist any which describes hands photoaging..Objective. To develop a photographic guide for assessment of dorsal hands photoaging. Materials and Methods: Nine characteristics were selected from SCINEXA score, to be used in the second phase where five experts determined four grades of hands photoaging severity. In the third phase another group of six experts classified 150 hands standardized photographs according to the severity grades. Statistical analysis was performed to assess expert agreement and photographs with best concordance were used to make the photographic guide. Results. Almost 100% agreement was found between experts for all the characteristics used to classified photoaging severity grades. 41% agreement was found between experts in the classification of 150 hands standardized photographs according to the severity grades. The best agreement was given for the grade 0 (57.2%). Discussion. Up today the photographic hand scales use crono and photoaging caracteristics to grade hand aging severity, while the photographic guide obtained in this study takes only the photoaging characteristics and thats why it is a reliable tool to assess hand skin photo damage and to evaluate effectiveness of photoaging treatments.
