Correlación entre la escala clínica de Apache II y la imagenológica de Balthazar en la evaluación de pancreatitis aguda severa en pacientes adultos de la Fundación Cardioinfantil entre 2008-2010
Early diagnosis in acute pancreatitis is key to adequately address the patient, to reduce complications. In assessing the severity of pancreatitis, it has the scan, useful in determining local complications, however, it is necessary to establish its correlation with the patient"s clinical course, assessed by the APACHE II that has several variables. METHODOLOGY: To determine the correlation between clinical scales and imaging in assessing severity of pancreatitis. For this, we reviewed medical records of the Cardio - Infantil of patients with acute pancreatitis are reviewed if they meet inclusion criteria, and descriptive statistical analysis used SPSS to establish correlation between variables. RESULTS: There is poor correlation between the variables, they are scales that assess different parameters, which makes these scales are complementary. DISCUSSION: We analyzed 189 patients from a population of 264, the rest did not meet inclusion criteria. The demographic characteristics and etiology of pancreatitis are similar to those described in other studies. Paraclinical were taken to define criteria Ramson, and abdominal scan was performed to search for local complications, according to the scale BALTHAZAR-Severity Index, and APACHE II clinical evaluation for clinical follow up. The criterion for admission to ICU is evaluated with the APACHE II can be periodically evaluated later correlation coefficients between the variables with the greatest impact on prognosis.
