Aims & Objectives: Post intensive care syndrome comprise multiple alterations, that are produced after critical illness. Instituto Roosevelt is the first hospital in Colombia, where a pediatric post-ICU program was created to reestablish functionality of the patient and caregivers. We aim to describe demographic, clinical, functional, and emotional characteristics of patients and caregivers from admission to 1-year follow-up. Methods: Pediatric patients with mechanical ventilation, inotropic support, multiple organ dysfunction syndrome, and post-cardiac arrest were included. Multiple evaluations were performed: cognition, functional, and feeding difficulties. In parents' anxiety, depression, caregiver fatigue, and quality of life. Results: 17 patients were included. We describe economic income (66% low-income) and precedence (urban area 88%). 65% males, mean of age 21 months (2-81). 70% had no comorbidities, 12% developed neurological alterations, acute kidney injury 47%, and 59% had shock. 35% of the patients had ARDS. Non-invasive ventilation was present in 62% (median 4 days) and invasive ventilation 71% (median 6 days). Mortality risk predictors were assessed, PRIMS (p50= 6) and PELOD (p50=2). None had previous functional compromise and only one patient had cognitive impairment. At admittance, caregivers report anxiety (33%), depression (16%), and mild caregiver burden (64%). Follow-up was accomplished in the first, third, and sixth month: 82%, 41% and 11% respectively. Functionality, anxiety and depression improved, but caregiver burden increased. Conclusions: This program has received a good welcome of caregivers. They've perceived support of healthcare professionals. It provides a closer follow-up and improvement of opportune care, in a country with difficulties of healthcare access. Keywords: Post-ICU, Functionality, pediatrics, PICS
