Introduction: Marchiafava-Bignami disease is characterized by rare presentation, where necrosis and demyelination of corpus callosum occur. It should be suspected in patients with chronic alcohol consumption, and its clinical presentation begins with neurological alterations. Alcoholism worldwide brings about 3.3 million deaths a year and Colombia is no stranger to this problem, with a prevalence in males. Description: We present the case of a 34-year-old man with a history of chronic alcohol consumption since his youth, who presented with an acute myocardial infarction with ST-segment elevation. Subsequently, he experienced a cardiac arrest with a deterioration of consciousness. Considering the suspicion of hypoxic-ischemic encephalopathy, cerebral magnetic resonance imaging was performed, revealing findings consistent with symmetric frontoparietal occipital leukoencephalopathy and involvement of the corpus callosum, indicative of Marchiafava-Bignami disease. Discussion: Chronic alcoholism in Colombia is a condition with a high incidence, particularly among the young population. This case represents an opportunity to showcase the clinical and radiological findings of Marchiafava-Bignami disease, providing valuable guidance and information to the medical and scientific community. As the existing literature is scarce and primarily consists of case reports and case series, this study aims to contribute with significant information to enhance the understanding of this rare condition.
