A 26-year-old female in her fourth week of pregnancy presented with uncontrolled high intraocular pressure (IOP) in her left eye. Her best-corrected visual acuity, IOP, cup-to-disc ratio, and average retinal nerve fiber layer thickness were 0.097 logMAR, 37 mmHg, 0.9, and 61 µm, respectively. Her central corneal thickness was 570 µm, and she had anterior synechiae, indicating previous uveitic attacks. In this case, gonioscopy-assisted transluminal trabeculotomy (GATT) was recommended and performed successfully. On postoperative day 1, IOP spiked (45 mmHg); however, it decreased to 13 mmHg three months later without medications. Thus, GATT can be an appealing surgical alternative, especially during pregnancy.
