Local burn wounds management should be aimed at stimulating adequate healing by creating a wound microenvironment that provides optimal healing with minimal pain and few side effects. It should also reduce the probability of infection, cause minimal discomfort to the patient and facilitate care; however, there is no consensus for optimal local treatment. Objective: to compare 1% silver sulfadiazine with hydrofiber dressing with silver from the beginning of its implementation in this institution. Method: the medical records of 138 pediatric patients between 0 and 18 years old with second-degree burns (superficial and deep) involving less than 20% of total body surface area and that were not associated with another type of trauma were included. The period considered was between January 2014 and December 2017, when the management process with the hydrofiber dressing with silver was being implemented. The percentage of patients management with a silver hydrofiber dressing is 65.2% (n = 90) and the remaining 34.8% (n=48) is treated with 1% silver sulfadiazine. The data collected, in addition to the epidemiological characteristics, were: number of times a wound is healed in the service and in the operating room under general anesthesia, infection status at each dressing change, epithelialization time, analgesics and antibiotics use time, presence of pain and itching, hospital stay and costs of care. Results: statistically significant differences were obtained in terms of epithelialization time, analgesic use time, hospital stay, and costs, all in favor of silver sulfadiazine. Conclusion: the results obtained contradict those of many studies previously carried out. Considering that the evaluation was made during the application of the hydrofiber dressing with silver, the results allow to establish the mode of use of these two topical medications and to adopt a training process to guarantee adherence to the institutional protocol, as well as to plan the performance of a randomized controlled clinical trial that allows to definitively determine whether or not there are differences between these two treatments.
