The treatment of esthetic areas has received a great deal of attention since the turn of the century. Nowadays, the key objectives of implant therapy are associated not only with successful osseointegration of the dental implant(s) and peri-implant tissue health but also with the achievement of esthetic harmony between the implant restoration(s), the peri-implant soft tissue, and the adjacent teeth/gingiva. Alveolar ridge preservation is a therapeutic option that has gained increasing projection in implantology in an attempt to preserve the alveolar bone ridge dimensions following tooth extraction. The use of facial photographs allows a better identification of potential discrepancies associated with esthetic sites, but some issues should be considered. Esthetic dentistry is related to facial, tooth aspects and soft-tissue aspects. The gingival architecture directly influences teeth harmony. Anatomically, the peri-implant mucosa presents a microstructure that is similar to the gingiva around natural teeth, basically formed by oral and junctional epitheliums, and a connective tissue attachment.
