P0183 / #1510: EXPERIENCE IN THE IMPLEMENTATION OF IMPROVEMENT STRATEGIES IN ADVANCED PEDIATRIC RESUSCITATION IN THE INSTITUTO ROOSEVELT. PARTICIPATION IN THE PEDIRES-Q COLLECTION DATA.
Aims & Objectives: We present a description of the experience in a pediatric hospital with the implementation of improvement processes in advanced cardiopulmonary pediatric resuscitation, in the the pediRES-Q study Methods: We describe the experience of our hospital and perception of the health care professionals in the implementation of high quality resuscitation strategies recommended since 2017 and in the frame of the pediRES-Q between August 2018 and February 2020. Training, debriefing and registration protocols of advanced cardiopulmonary resuscitation were implemented in the 29 cases registered in the database. Evaluation was made by a survey which determined personal perception and satisfaction with the health care provided in the frame of adherence to protocols and quality of the advanced pediatric resuscitation. Results: The collected data presents a time of response to initiate CPR under 1 minute in 80% of the cases. Return of spontaneous circulation was lower in patients under 1 year. Survival to hospital discharge was lower than the other centers in the pediRES-Q. Health care professionals overestimated the quality of the adherence to protocols before, during and after response of the cardiac arrest. Conclusions: Results about performance were similar to the statistics found in the literature. 80% of the cardiac arrests were presented in the ICU or surgical room facilitating identification and shortening of time response. Survival to hospital discharge was lower secondary to neurological comorbidities of our population. Overestimation of adherence to protocols can be explained by evaluation activities, risk mitigation strategies and permanent feedback. All processes must be systematized to improve health care.
