Ventricular arrhythmias (VA) originating from the left ventricular (LV) summit and intramyocardially within the interventricular septum pose a serious challenge to catheter ablation (CA), as myocardial thickness, epicardial fat, and coronary vessels impede appropriate radiofrequency (RF) energy delivery to the target areas. Several strategies have been proposed to effectively eliminate these VA.1–5 Nonetheless, each of these is associated with a series of limitations and potential complications, and until now no consensus exists on the optimal ablation strategy for VA originating from these anatomic sites.
