Introduction: Foreign bodies may be dormant and remain in the soft tissue for years without causing damage to adjacent structures, but it can make a granulomatous foreign body reaction. A granuloma is a distinct, compact microscopic structure composed of epithelioid-shaped macrophages typically surrounded by a rim of lymphocytes and filled with fibroblasts and collagen. Granulomatous inflammation of oral soft and hard tissues is an uncommon occurrence. Patients present often with oral pain and signs of inflammation with purulent discharge. However, sometimes it has been seen asymptomatic, becoming a difficult diagnosis with several differential diagnoses as sarcoidosis, Crohn disease, and others.
