Hodgkin lymphoma is a malignant tumor originating from lymphocytes that frequently presents as a nodal lesion and rarely involves extranodal sites. The literature reports only eight cases of primary Hodgkin lymphoma arising in oral mucosa and one of them in the palatal mucosa, in the absence of nodal disease. We report a 43-year-old white female patient with an unusual manifestation extranodal of primary Hodgkin lymphoma in the hard palate with clinical aspects similar of other infections cutaneous mucosal diseases. Analyses of infectious agents resulted negative. An incisional biopsy showed lymphoplasmocitary infiltrate with presence of Hodgkin and Reed-Sternberg cells that exhibited immunoreactivity for CD30 and Fascin and nonreactivity for CD15, CD45, CD56, CD20, CD3, and TIA1, supporting a diagnosis of classical Hodgkin lymphoma clinical stage according to Ann Arbor I-A. The patient was submitted to three cycles of chemotherapy (ABVD, doxorubicin hydrochloride [Adriamycin], bleomycin sulfate, vinblastine sulfate, and dacarbazine) and there was no evidence of diseases at 15 months of follow-up.
