Post-Chikungunya Chronic Inflammatory Rheumatism: Results From a Retrospective 12-Month Follow-Up Study of 171 Cases in La Virginia, Risaralda, Colombia
Background.There are limited studies in Latin America regarding the chronic consequences of the Chikungunya virus (CHIK), such as post-CHIK chronic inflammatory rheumatism ( pCHIK-CIR).We assessed the longest follow-up cohort so far of pCHIK-CIR in Latin America, at the municipality of La Virginia, Risaralda, a new endemic area of CHIK in Colombia.Methods.We conducted a follow-up cohort retrospective study in Colombia of 171 patients diagnosed with CHIK that persisted with pCHIK-CIR after a minimum of 9.1 months and up to a maximum of 12.6 months (February 2015-March 2016).pCHIK cases were identified according to validated criteria via telephone.Those with other arbovirosis during follow-up were excluded.Results.Of the total CHIK-infected subjects, 78 (45.6%) reported persistent rheumatological symptoms ( pCHIK-CIR).All of these patients reported joint pains (chronic polyarthralgia, pCHIK-CPA), 43.9% morning stiffness, 38.6% joint edema, and 19.9% joint redness.Of all patients, 21.1% required and attended for care prior to the current study assessment (2.9% consulting rheumatologists).Conclusion.According to our results, after nine months almost half of the patients with CHIK persisted with chronic rheumatologic sequelae.In La Virginia, an initial follow-up assessment of up to 6.5 months showed a persistence of 53.7% in a group of 283 patients.These results support previous estimates obtained from a meta-analysis that included studies in La Reunion (France) and India, among other countries, and are consistent with the results previously published from the Colombian cohorts in La Virginia (Risaralda), Venadillo (Tolima) and Since (Sucre).Disclosures.
