Introduccion: la hiperplasia lipomatosa de la valvula ileocecal (HLVI) es una entidad rara, definida como una proliferacion difusa e infiltrativa de tejido adiposo maduro a nivel submucoso. Caso clinico: paciente masculino, de 67 anos que acudio al Hospital Universitario del Caribe, Cartagena, Colombia, con cuadro de dolor abdominal agudo. Se realizo diagnostico de abdomen agudo secundario a apendicitis aguda, sin descartar posibilidad de neoplasia de colon. Se encontro masa en ciego y se realizo hemicolectomia derecha, omentectomia, anastomosis ileo-transverso termino terminal. Buena evolucion operatoria. El resultado de la pieza quirurgica fue: hiperplasia lipomatosa de la region cecal. Conclusiones: la HLVI suele ser asintomatica, pero cuando mide mas de dos centimetros puede causar obstruccion intestinal, simulando procesos inflamatorios o malignos. SUMMARY Introduction: The lypohyperplasia of the ileocecal valve (LHIV) is a rare disease defined as a diffuse and infiltrant proliferation of mature adipose tissue to submucose level. Case report: 67-year-old male patient that attended to the Hospital Universitario del Caribe, Cartagena, Colombia, with symptomatology of acute abdominal pain. The diagnosis of acute abdomen caused by acute appendicitis was done without rejecting the possibility of colonic neoplasm. A mass in cecum was found and a right hemicolectomy, omentectomy and termino-terminal ileotransverse anastomosis were carried out, with good evolution after the surgery. The result of the surgical piece was: lypohyperplasia of the cecal area. Conclusions: The LHIV tends to be asymptomatic but when it measures more than two centimeters, could cause intestinal obstruction pretending inflammatory or malignant diseases.
