Introduccion: el agrandamiento gingival es el aumento del tamano de la encia causado por diversos factores, entre los cuales se encuentran medicamentos, principalmente antihipertensivos, inmunosupresores y anticonvulsivantes. Entre los primeros, el nifedipino, un antagonista del calcio, es uno de los mas frecuentemente senalados. Caso clinico: paciente masculino de 62 anos, con hipertension arterial tratada con nifedipino por mas de quince anos, quien presento agrandamiento gingival cronico en la zona de los incisivos laterales y caninos del maxilar superior. Se realizo gingivectomia a bisel externo, con electro bisturi, con evolucion adecuada y buena cicatrizacion. Conclusion: el agrandamiento gingival segundario a nifedipino, amerita tratamiento, que incluye suspension del medicamento e higiene oral. En algunas circunstancias se debe realizar gingivectomia. SUMMARY Introduction: the gingival enlargement is the increase of the size of the gingival caused by diverse factors, between which are the drugs, mainly the antihypertensive, immunosuppressive and anticonvulsant medications. Between the first ones, the nifedipine, an antagonist of the calcium, is one of the most frequently indicated. Clinical case: A 62-year-old-male patient with arterial hypertension treated with nifedipine for more of fifteenth years, who presented chronic gingival enlargement in the area of the lateral incisors and cuspids of the maxilla. External bevel gingivectomy with electric scalpel was carried out, with appropriate evolution and good healing. Conclusion: the gingival enlargement caused by nifedipine merits treatment that includes suspension of the drug and oral hygiene. In some cases, gingivectomy must be done.
