Twiddler´s syndrome is a rare complication of cardiac conduction devices; despite its low frequency, it should be considered when evaluating patients with suspicion of pacemake´s dysfunction. We report the case of a 62 year old man who consulted Hospital Universitario San Vicente Fundación, in Medellín, Colombia, with decompensated heart failure due to dislodgement of the left ventricle lead of his recently implanted biventricular pacemaker. The patient improved with the placement of a new device.
