Introduction: The irritable bowel syndrome (IBS) is a frequent cause of children consult, including an important psychosomatic component. The objective is to determine the psychological manifestations and characterize the familiar dynamics of children with IBS. Methods: Descriptive and observational study (n=15, age 27±15 months, 8 males), children who were going to consult at the Hospital Universitario del Valle “Evaristo Garćia”, in Cali, Colombia during 8 months. A semi-structured survey was made. Results: 9 (60%) patients were infants; 11 (73.3%) were born through a non planned pregnancy; 10 (66.7%) had been toilet training before two years old; 9 (60%) were single children and had separated parents, respectively; 8 (53.3%) with mothers <25 years old and “sickly” children, respectively; 7 (46.7%) “restless” and having “feeding problems”, respectively; 6 (40%) with breast-feeding > 6 months and walker before one year old, respectively. These observations allowed to understand the familial dynamics and its possible link with physical symptoms. Conclusion: A notable familiar dysfunction exists, with presence of chaotic and hostile dynamics in children with IBS, being the recurrent characteristics as follows: single son infant “sickly with feed problems, restless and early walker”, with separated parents, mother <25 years old, with background of non planned pregnancy and with early toilet training. The hostility demonstrates the familial chaos and the parents’ emotional immaturity; this joined to the parents separation has repercussions on the maternal role due to the lack of paternal and conjugal support, resulting in the alimentary restriction in the kid as a manifestation of reproach and compensatory mechanism.
