The objective of this article is to present the foundations of a new conceptual Knowledge Management Model. The model is product of investigation, and allows the development of activities to implant practical solutions of knowledge management in organizations. The model includes five phases, each one, having their own set of products and activities as well as their achieving techniques and means of quality. These techniques include the use of lessons learned, best practices, learning processes and communities of practice, among others. In the final sections, an evaluation of the knowledge management model is made, and finally conclusions are presented as well.
