<b>Background:</b> patient´s adherence to pulmonary rehabilitation (PR) remains a challenge despite increasing research. <b>Aim:</b> To identify enablers and barriers for adherence to PR in COPD patients using a theoretical model. <b>Methods:</b> We conducted a scoping review using Medline, Ovid, Science Direct, EMBASE, EBSCO and ISI web of science, we included studies with COPD patients enrolled in PR programs that measured adherence, barriers and enablers with clear criteria. We used WHO model on patient adherence in order to classify enablers and barriers reported in five dimensions: patient related, disease related, treatment related, social and economic aspects and health care system and staff. All studies were critically appraised using tools according to their design. <b>Results:</b> We included 20 studies in this review, we found that disease related and patient related factors accounted for 70% of factors reported in the literature where as treatmet related, social-economic and health care system factors were less reported. Barriers to patient´s adherence were lack of motivation, lack of independence, exacerbations, depression and anxiety, and transportation difficulties. The more frequently reported enablers were perception of PR benefits, knowledge about COPD, support from health care staff, and improvement in physical and mental function. <b>Conclusions:</b> To improve patient´s adherence, PR programs must engage patients from the referral, improve educational components of the program, support patients during their journey and create support networks to facilitate their assistance. There is a need for increase research related to social-economic and health care system aspects of adherence to PR programs in COPD patients.
